Mandatory Full-Time staff meeting for health, dental and life insurance:
The new plan year is effective 12/01/2017 and all eligible staff need to sign either an enrollment or waiver form. Coverage does not continue automatically. All full-time eligible staff (60 hrs or more per pay period) are required to attend either the 1pm or 3pm session on Wednesday, 11/08/17, but all staff members who would like more information about our full-time benefits are invited to attend.
Part-Time & Full-Time staff eligible for voluntary insurance (40 hrs or more per pay period):
Our Colonial Life representative will be here all day Wednesday, November 8th from 10:00 am – 5:00 pm to meet with staff and provide more information. Voluntary benefits include short-term disability, accident insurance, etc. Please meet with him if you currently have voluntary insurance coverage or are interested in the voluntary benefits available.
Those who qualify for benefits are required to sign either an enrollment or waiver form. All forms are due by 11/15/2017.
